Chris#2
try here - The Whisky Exchange : Whisky Shop, Single Malt Scotch Whisky, Whiskies of the World, Rare Whisky - I bought a vintage bottle for my father-in-law's retirement. Dunno if they go down to the exact day, but worth a look.

Frosty Jack
To be honest I doubt you'll get anything for a specific date from anyone. There are two dates relevant to a bottle of whisky, the distilled date (i.e. when it is put into the barrel) and the bottled date - but these seem to be usually just recorded by nothing more specific than year. The difference between the two is the age of the whisky. An idea I've seen elsewhere is to get a bottle which was distilled in the father's year of birth, and bottled the year the baby was born. Tricky, depending on how old the father was when the baby was born, but could be do-able...
french_disko
Gordon & Macphail do personalised labels for whiskys. Although the liquid inside might not be specific enough for you, the bottle - which is more likely to last longer than the whisky - would always be special.
