I'm not sure what the 6th one is. The little nick in the live and the messy neutral wire are both sloppy but not dangerous.
EDIT: The live wire is too long, if that pops out it's long enough to reach the ground terminal.

*No prizes for killing people and/or destroying equipment I'm afraid.
